Generally speaking, patients start at a dose of one 50 mg/20 mg capsule (this indicates 50 mg of xanomeline and 20 mg of trospium chloride), two times daily. They stay at this starting dose for at least two days before increasing to a 100 mg/20 mg capsule twice daily. They stay at this dose for at least five days. From there, their doctor can increase their dose up to the maximum dose of 125 mg/30 mg twice daily.
Doctors typically recommend that their patients take Cobenfy at least an hour before or two hours after a meal. The capsules should be swallowed whole without breaking or emptying.
